Personalized and Skilled Care

The staff at our nursing center in Grantsburg offers long-term care that is personalized for every resident. We do this by developing a care plan upon admission, which is based on the resident's desires/goals and care team assessment. We encourage families to participate in continuing care conferences where recovery and/or rehabilitative progress is reviewed, and recommendations are made.

The Continuing Care Center's goal is to help each Grantsburg resident achieve his/her maximum level of independent functioning. Although a resident may not be acutely ill from an injury or illness, the restorative and/or rehabilitative services regularly provided help continue the recovery process or maintain the current physical condition.

Only physicians can write admission orders, but family members or social service agencies may also request referrals by calling the Continuing Care Center Social Workers at 715-463-7278.

Accreditation
The Continuing Care Center in Grantsburg is certified by Medicare and Medicaid and is licensed by the Wisconsin Department of Health and Human Services. We are also a VA certified care facility.
