Burnett Medical Center’s Continuing Care Center in Grantsburg is a long-term and rehabilitative care facility that provides essential health services. Our goal is to foster hope, harmony, and healing in a home-like environment.

We encourage everyone receiving care to make our center a “home” away from home, even for a brief stay. Family photographs, furniture, and quilts are the special touches of home commonly found in individual rooms.

Personalized and Skilled Care
The staff at our nursing center in Grantsburg offers long-term care that is personalized for every resident. This is achieved by developing a care plan upon admission, which is based on the resident’s desires/goals and care team assessment. We strongly encourage families to participate in conferences where recovery and/or rehabilitative progress is reviewed, and recommendations are made.
Our center’s goal is to help each Grantsburg resident achieve his/her maximum level of independent functioning. Although a resident may not be acutely ill from an injury or illness, the restorative and/or rehabilitative services regularly provided help continue the recovery process or maintain the current physical condition.
Insurance
Residents and/or family members are encouraged to discuss their particular health insurance coverage and payment options with the Social Services Department when admission is being planned.
Admission Process
Only physicians can write admission orders, but family members or social service agencies may also request referrals by calling the Social Workers at 715-463-7278.
Accreditations
Our center is certified by Medicare and Medicaid and is licensed by the Wisconsin Department of Health and Human Services. We are also a VA certified care facility.
